AR-11 SR, Alien's Change of Address Card

Purpose of Form

To report changes of address of aliens subject to Special Registration requirements. Special Registrants must submit this form within 10 days of a change of address.

Disclosure: Failure to report a change of address may result in a fine, imprisonment and/or removal. 8 U.S.C. sections 1227(a)(3),1306. Failure to comply may also jeopardize your ability to obtain a future visa or other immigration benefits.

Special Instructions

While it is not required, USCIS recommends that aliens filing a Change of Address keep a copy of the form for their records and obtain a receipt showing the date and address to which the form was mailed.
