Federal regulations 8 CFR 214.3(g)(1) require designated officials of your school to supply this Service, upon its request, with specific information and documents about your school's F-1 and/or M-1 students. Please furnish the information and/or documents about the above student which are checked below:

____Current address and telephone number.

____Status, i.e., whether full-time or part-time

____Course load.

____Date of commencement of studies.

____The documents related to the student's admission to the school, as specified in 8 CFR 214.3(k) .
